Case Manager

The Veterans Multi-Service Center (VMC) exist solely to "serve those who have served" this great nation, our Veterans. We are a nonprofit organization that offers comprehensive services to aid our Veterans are homeless, needing skills to obtain a new career, etc.. As the Case Manager you will be deeply involved with aiding our Veterans to get back on their feet.

 

Position Title: Case Manager

Type of Position: Full-time

Location: Coatesville, PA  

Reports to: Senior Case Manager

Requirements: Driver’s License

Education: Bachelor’s degree preferred. Equivalent education and experience considered.

Travel: 15-30% 

 

Position OverviewThe Case Managers provide Case Management for approximately 15-30 Veteran households at a time, supporting development and achievement of individual housing stability plans. 70% of the Case Manager’s work effort should be to case management and approximately 30% with outreach.

 

Key Responsibilities:

  • Consulting with potential clients to assess needs and determine program eligibility, ensuring that clients obtain needed supportive services
  • Working with clients to create individualized housing stability plans with client-specific goals.
  • Providing ongoing support and expertise through assessment, planning, implementation, and overall evaluation of the Veteran’s needs as drawn from his/her goal/action plans.
  • Obtaining information contributing to the client’s situation and evaluating these, along with the client’s capabilities/skills.
  • Collaborating weekly with the Housing Coordinator, Social Worker and Income Stabilization Specialist on action/service plans, coordinating approaches for successful outcomes.
  • Following up to determine quantity and quality of service provided to the Veterans and the status of his/her care
  • Conduct outreach to identify and provide services for target populations
  • Develop effective plans for clients, in coordination with the Social Worker
  • Provide Case Management with a focus on “housing first”
  • Support and execute fundraising events

  • Assist clients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  • Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.
  • Identify yourself in a pleasant and positive manner.

  • Computer Skills: Including but not limited to MS.

Other required skills: The ability to communicate with all levels of the organization. This position also requires a solid, self-directed individual who can excel both independently and as a team player in a fast-paced, entrepreneurial environment.   Candidates must be able to complete tasks according to accepted levels of quality. Experience in a growing entrepreneurial company is preferred. Excellent work ethic, enthusiasm and positive energy.
