Case Manager (Transitional Housing)

 

Case management begins immediately upon admission to the VMC Transitional Housing Program.  The new resident is required to meet with their Case Manager for the establishment of both short and long-term goals relating to their eventual discharge into the community. The Case Managers will assist in developing and implementing the Individual Service Plan (ISP) which may include medical and mental health services, substance abuse recovery, personal safety, benefits, budgeting and job and housing counseling. 

 

KEY RESPONSIBILITIES:

  • develops individual service plans on long term housing needs and assistance plan adherence with reviews and updates every 30 days.
  •  

  • places program discharges in community based or independent housing and assist with placement in personal care or nursing homes those veterans who require this housing need.
  •  

  • holds monthly community meetings addressing living arrangements and fostering consideration of "neighbors" to veteran residents.
  •  

  • assesses admissions for employment ability, to include VA Vocational Rehabilitation eligibility and set short and long-term goals on employment and or income options with monthly updates.
  •  

  • provides case management with an initial comprehensive assessment and develop service plans and goals with monthly case management meetings for updates and to assess eligibility and assist in applying for SSI, SSDI, VA entitlements, benefits and pensions.
  •  

  • coordinates, through case management, an active behavioral health treatment plan when appropriate. 
  •  

  • provides educational presentations related to functional life skills, time management, anger management, communication skills, values clarification, budgeting, independent living and parenting skills.

 

QUALIFICATIONS:

One to five (5) years’ experience in a drug and alcohol recovery program and mental health environment.  Minimum one-year case management experience preferred. Knowledge of veteran entitlements.  Working knowledge of principles of 12-step based recovery programs

 

Bachelor’s degree preferred in human services or related field.  Equivalent education and experience considered.

 

Compensation consideration commensurate with experience and qualifications.
